I had a similar experience on a fire escape. Just sunk down into it about half way. Both levels. (At a spawn building)

Wooden mezzanine floors (not the ground, but up the stairs) at the Ashford factories (any of them).

You are OK on metal floors and stairs, but the wooden floors drop you straight onto the ground floor (I reported this already ;))
