Mission Pending Bug - Additional Info Noted:

I have been having the mission pending bug much too often.

I have noticed that when I die and get the kill-cam that I am having to hit the "OK" button twice versus just once when the timer finishes the count down for a normal death exit.

Every time I have to hit OK twice I am rewarded with the mission pending bug. I don't exactly remember though if I hit the ESCAPE button during the despawn count down or not. Just notice that sometimes I have to hit OK twice.

When I am required to hit the OK button once then all is well.

(yah, vague and repetitive....)

Not vague at all. But it i a symptom not the cause unfortunately.
